The standard is clear. It's up to the designer to follow the standard, not vice versa. Dimensioning the CS diameter to either the major or the minor size on a cylindrical surface, like you would do on a flat surface, will not give you a fastener that is below the entire surface. You'd have to have a counterbore at the top for that to happen. This should be noted if that's the design intent.

I can not find how it is dimensioned in ISO but on drawings should be mentioned which standards are used.
Then designer should follow these rules to ensure later understanding. But standards are changing quietly often.

Standards should reflect practise in process / make quick measuring on machines easier and more reliable, aka. more precise.

My personal opinion - ASME in this case did wrong job.
