The second message usually means that scanning speed was too high for the form deviation of the part. Since this is not the case in stylus qualification, you should first check if the master probe or qualification sphere is loose or if there's any damage.

The first message may appear due to maximum allowed sigma values being set in Calypso (check CMM tab -> Stylus system -> button "Set Limit Values"). If they are exceeded, this also hints at a possible problem with the probe or the sphere.
