You should be able to use the top surface as your z origin, and the OD or ID as x- and y-origin. Then I'd try either the top surface as spatial rotation (z-axis) and some feature on the horizontal arm (the hole, maybe? or a circle on the OD) as planar rotation, or the horizontal OD cylinder as spatial (x-axis this time) and the top surface as planar. You're measuring the planar rotation feature as a plane, not just a single touch point or something like that, right? I work at a place that made medical components for other people, and that looks like the head of a dental tool. Don't remember the company or the name of the tool anymore.
