Adding Signature to report header

I have a question on adding a signature line to the Pi Web report header.  I am new to Pi Web and am trying to figure it out as I go.  I see that, in the measurement plan, I go to Measurement tab.  I click on Multiple Report.  This brings up the Definition of multiple report.  Pi Web is clicked, and it is reporting.  I go to Select report header parameters, and there is one that says Signature.  I click on the run radio button.  It comes up to add the signature when running the report, however it does not adding the signature of who ran the part to the report header.  What am I doing wrong here and how do I add this to the report header?  Thank you for the help with this.

There should be two radio buttons, one is for the dialog box, and the other should be for it to show in the PiWeb report.

I tried clicking both and no luck.  I think I need to add the text field to the report header, then it may work, but I am unsure.  Like I said, I am new to PiWeb and not really sure what I am doing.  Just trying to learn as I go on my own.  Thank you for the help though.  I tried clicking 1 of the radio buttons, then both and neither worked for what I wanted.  Will keep pulling my hair out trying to figure this out.
