Copy with References and Paste with References (Issue)

I never noticed this before.
I'm building a large PPAP program from many operation programs.
In several operation programs I use "Patterns" for some large surfaces, there should only be three total patterns in use.
I have found 15 patterns in my PPAP program after COPY/Paste with References (although I do not continuously paste the same feature over an over).
They just keep building up.

Copy with References does the same to me. I expected the dialog for Overite, rename, etc.
